Men's Hockey Apparel Store: Extended through September 6

The Concordia University Wisconsin Men's Hockey store is open for fans to get their Falcons gear until Thursday, September 6. To get your CUW Men's Hockey gear for the 2018-19 season, follow these simple steps below. 

1) Visit KTforms.com from now until September 6. 

2) Type in your Access Code (CUHKY) in box at top of page & click "GO".

3) Select merchandise and add to shopping cart. 

4) Once shopping is complete, click CHECKOUT & either register as a New User or log in with previously registered information.
